Registration for sports must be completed by both parent and athlete. Athletes must have a valid physical on file with the athletic department, ALL online electronic forms submitted, ASB and sports participation fees paid to be eligible for participation.

In order to participate in the Spring Sports Season (6th/7th/8th Boys & Girls Track & Field), all online paperwork must be completed by Thursday, March 28th to be eligible for the first practice on Monday, April 1st.

Registration is done online through FinalForms. Your information is saved from season-to-season and year-to-year, making the registration process quick and easy. You will be required to sign your forms once per year and after any update.

If you are not registered as a Tolt Middle School student yet:

6th-8th graders not attending Tolt Middle School who live within the Riverview School District boundaries must contact our registrar, Carmen Hobbs at hobbsc@rsd407.org, to register as an ancillary student in order to participate in sports. Download the Ancillary Services Request hereAncillary students are required to submit grade reports on the dates established at the beginning of each sport season. Deadlines for ancillary registration for sports: Fall - Thursday 8/24/23, Winter I - Thursday 10/12/23, Winter II - Wednesday 1/4/24, Spring - Thursday 3/21/24. Ancillary status does not carry over, it must be renewed every school year.

IMPORTANT REQUIREMENTS TO BE ELIGIBLE FOR 1ST PRACTICE

The following checklist is provided to assist student-athletes in meeting the requirements to participate in any activity:

- Register online through FinalForms 

- Complete Physical Examination Form on file with the Athletic Department

o Physical paperwork is valid for two years from the date of examination and must be valid for the duration of the entire sport season.

- Purchase ASB Card through the Optional Fees tab in eFunds ($25)

- Pay Participation Fee through the Optional Fees tab in eFunds ($85 per sport, $170 max per student per year)

- All outstanding fees on the student's eFunds account must be paid before being cleared for athletic participation.

- To apply for sports participation fee waiver please complete both the Request for Fee Assistance form and Free/Reduce Meals form which can be obtained online at the Riverview School District website.

 

FinalForms Instructions

You will need the following information to allow for accurate completion of your online registration:

- Basic Medical History & Health Information

- Insurance Company & Policy Number

- Doctor & Dentist Contact Information

- Hospital Preference

Registering a Student

- Visit FinalForms 

- Click LOGIN under the Parent icon.

- Click INCOMPLETE FORMS.

- Select the sports for which you would like to register. Your selection can be changed at any time prior to the registration deadline.

- Complete each form and sign your full name in the Parent Signature field at the bottom of the page. After signing a form, click SUBMIT FORM and move on to the next form.

- You will see a “Forms Finished” message when all forms are complete.

- If required, an email will automatically be sent to your student's school email, prompting them to complete the forms requiring their signature.

- Click MY STUDENTS when you are done. Repeat steps 3 - 6 for additional students.

- If you need to update your information, click the UPDATE FORMS button.

Grades

Grade checks are run throughout each sports season.

 

If you attend any other school for ANY classes, you must provide the Athletic Department with your grades from that school weekly.

 

All of the above items must be completed and submitted to the Athletic Department by the above DUE DATES to be eligible for 1st turn-out.

Additional Information:

- Students may not practice until their current physical is uploaded in FinalForms.

- On the first day of practice, students should come prepared to participate and meet in the locker room after school to get changed. Coaches will instruct students from there as to where to go and what to do.

- Lockers will be assigned to those students not currently in PE class.

- Per WIAA rules, students must attend eight (8) practices to be eligible for competition (games/meets).

- All student-athletes will be issued a uniform for competitions. The uniform will be on loan during the season and must be returned clean at the end of the season.

- Student athletes will be bussed to and from away games. Please make prior arrangements with the coach and/or Athletic Director if your student will not be riding the bus back to Tolt after a competition or if your student will miss any practices or competitions.

- Tolt has a no cut policy so everyone can participate as long as they keep up their grades. Students must be passing all their classes during the season to remain eligible to practice and play.

- 8th graders who plan to play fall sports at Cedarcrest High School must have passed all of their classes at the conclusion of their 8th grade year in order to be eligible for fall activities and sports. Failure to do so means the student/athlete will be able to practice but not compete for 5 school weeks. If after 5 weeks, the student/athlete is not passing all classes, they shall be ineligible for the remainder of the season.
